Purpose of Project:
To Educate the Haitian Community on Advance Care Directives using the conversation of your life (COYL) tool kit

Methodology:
This project was supported by the evidence and by the knowledge to action model. The project had a sample of 12 participants (N=12) from the Haitian community which formed the setting for the project. The Advanced Care Plan Engagement Survey (ACPES) was used which measured readiness to engage in ACP

Results:
Scores as low as 4, indicating no engagement through 16, indicating readiness to engage in ACP were collected at baseline, 1 week after the educational workshop and at 2 weeks after baseline during a booster session. A statistically significant improvement in the mean ACPES score results was shown from baseline to week 1 and at the booster. The results of this project addressed the community’s needs around EOL and demonstrated how education could close the gaps of cultural bias.

Implications for Practice:
The project has implications for nursing practice as nurses have the ability to connect across the continuum of care and are viewed as the most trusted profession.
