DescriptionPurpose of Project: The purposes of this project are to increase awareness in the patient population of the need to transition with a basic skillset to ensure their health in the future as an adult patient as well as use data from this project to develop a transition program for this practice that will be uniquely tailored to the needs of the population.
Methodology: This project used a cross-sectional survey approach to evaluate readiness to transition to an adult rheumatologist in a large pediatric rheumatology practice in northern New Jersey. Using the Transition Readiness Assessment Questionnaire (TRAQ) tool an evaluation was conducted of patient knowledge of disease for transition to adult care, self-management skills of participants, and use of health care services for rheumatic disease management.
Results: Knowledge gaps were identified in the following domains: appointment keeping, managing medication, and tracking health issues. A plan for transition readiness programming to improve patient health outcomes based on patient feedback regarding transition from the TRAQ tool was created and provided to the project site via a presentation.
Implications for Practice: The results of this project indicate that interventions aimed at improving self-management both of their health and the appropriate use of health systems should be standard in transition readiness programming. This and the introduction of formal transition programming will ultimately lead to improved health outcomes.
