TitleAdvancing health equity: identifying the factors that impede and facilitate the practical application of HBOC screening within underserved communities
DescriptionBackground. Genetic counseling and testing (GCT) improves patient outcomes by informing the cancer management and screening for people with hereditary cancer. Unfortunately, there are a lack of screening and referrals for GCT coming from the Community-Based Organizations (CBOs) that serve Latinas. In order to increase the number of referrals CBOs are able to provide, the barriers and facilitators impacting the referral process must be understood. Guided by an expanded CFIR framework, the goal of this paper was to (1) identify barriers and facilitators for HBOC screening and referral at CBOs who provide services to underserved Latino populations before implementing the screener and (2) engage CBOs to select and adapt a validated HBOC screener to implement. Methods. We partnered with four CBOs that provide healthcare services to the Latina community and conducted focus groups in which we discussed each sites’ awareness of HBOC, current GCT screening and referral practices, and barriers/facilitators to screening and referral. The research team also presented five validated HBOC screeners so that the participants could select one to implement, and they discussion focused on planned implementation at each site. Results. Several important barriers to GCT screening and referrals were identified, including themes of health equity, the cost of GCT, and the lack of Spanish-language materials and providers available to patients. Facilitators included the connection CBOs had to other organizations able to support their screening and referral efforts. Discussion. Findings from this study can inform the development of implementation toolkits for other community settings to implement HBOC screening and advance health equity.
