DescriptionPurpose of the Project: The purpose of this project was to examine Post Intensive Care Syndrome (PICS) risk factors and their prevalence in the intensive care unit, increase provider awareness and recognition of PICS, implement a screening process in the critical care setting, and record referrals to a PICS Clinic.
Methodology: Pre- and post-intervention retrospective chart reviews were conducted to establish risk factor prevalence and initiate referrals through screening tool implementation.
Results: Of 26 patients screened, 54.54% (n=12) were male and 45.45% (n=10) were female. A total of 84.62% (n=22) qualified for a referral with 59.09% (n=13) receiving referrals according to the protocol. Of 26 patients screened, 57.69% (n=15) met the sepsis criteria, 3.84% (n=1) met the cardiac arrest criteria, 19.23% (n=5) met the previous psychological disorder criteria, 30.76% (n=8) met the 4 or more ventilator days criteria, and 42.30% (n=11) met the positive CAM-ICU criteria. In the intervention group, 15.38% (n=4) patients met zero criteria, 61.53% (n=16) met one criterion, 15.38% (n=4) met two criteria, 7.69% (n=2) met three criteria, 0 patients met four or five criteria.
Implications for Practice: This project illustrates critically ill patients at risk for PICS can be identified for early referral in the acute care setting in order to promote implementation of coordinated care.
