DescriptionPurpose of Project: Obesity is one of the leading public health problems in the world today withincreasing morbidity and mortality from obesity-related comorbidities and preventable complications. The use of behavioral interventions such as the 5 A’s framework of obesity management is recommended by national healthcare organizations that found evidence of clinically significant improvements in weight loss and behavior change outcomes. Despite the evidence-based recommendations, there is a gap in the clinical practice of obesity management. The purpose of this quality improvement project is to improve health outcomes and quality of life in overweight and obese adults by promoting weight loss and behavior change using behavioral interventions.
Methodology: This quality improvement project used a longitudinal approach with qualitative and quantitative data collection. The sample was composed of 13 English-speaking participants, male and female, between the ages of 18 and 70 years old, who were overweight or obese, and had an email account. The intervention was implementing the 5 A’s framework along with a weight loss strategy that took place once a week in a 1-hour webinar, for 8 weeks. A questionnaire was administered and collected electronically during week 1 (baseline), week 4, and week 8 after the weekly webinar meeting.
Results: Both weight and BMI showed a statistically significant decrease over the 8-week study period (p<0.001 for both). Participants lost an average of 8.96 ± 5.68 pounds and had an average decrease in BMI of 1.45 ± 1.00 points. At least half of the participants (53.8%) reached the weight reduction goal of at least 5% of their initial body weight. Participant behaviors related to exercise, diet, and psychological habits showed statistically significant improvements over the study period as well.
Implications for Practice: This study sheds light on the gap in clinical practice that overlooks the need for healthcare providers to address and manage obesity as a chronic disease. Recommendations for healthcare providers to use behavioral interventions like the 5 A’s framework should be implemented into clinical practice. Further research is warranted on improving obesity management along with a change in healthcare policy to see changes in practice.
