DescriptionPurpose of the Project: The purpose of the project was to test a quality improvement (QI) approach consisting of a multimedia education/accountability bundle on a sample of prediabetic patients in a primary care setting. Methodology: The intervention introduced the use of tracking apps, diabetes prevention bundle and telephone check-ins by primary care staff over the course of 9 to 12 weeks. Results: As expected all participants saw positive change with more frequent tracking predicting greater decrease in the three measured variables. Diet tracking provided the most significant weight, BMI and A1c changes. Results showed significant marker reduction with A1c t(40) = 5.869, p < .001), BMI t(40) = 5.849, p < .001), body weight t(40) = 5.216, p < .001). Implications: Standardized education and accountability bundles such as this are at the forefront of ideal and effective prediabetes care. Use of such tools better prepares staff for fully intervening, and dramatically increase the chance patients can avoid this potentially fatal disease.
