DescriptionPurpose of Project:
This pilot project focused on implementing a same day discharge (SDD) protocol after elective percutaneous coronary intervention (PCI) at a hospital in northern New Jersey. The Society of Cardiovascular Angiography and Interventions (SCAI) recommendations supporting SDD as a safe alternative to overnight stay was the basis of this pilot project.

Methodology:
This pilot project aimed to improve the SDD process after elective PCI. Two groups of participants were followed, SDD and overnight stay (ONS), and three outcomes were measured: patient satisfaction, readiness for discharge, and occurrence of adverse cardiovascular events within 30-days of the procedure by telephone. The site was a 255-bed community hospital in northern New Jersey where approximately 1,000 cardiac catheterizations are performed annually.

Results:
Patient Satisfaction: Participants in both groups were satisfied with their overall experience, discharge and medication instruction explanation. However, four participants required additional explanation about the reason for taking anti-platelet medications.

Readiness for Discharge: All participants felt ready for a SDD if given the choice. No participants in the ONS group felt an ONS was necessary.

Adverse Cardiovascular events: There were no participants in either group who experienced adverse cardiovascular events or required further treatment within 30-days of the procedure.

Implications for Practice:
These results impacted clinical practice at the site by testing a SDD protocol that includes a more thorough discharge assessment, but also standardize the way nurses provide medication education to all patients after PCI. Based on patient satisfaction and readiness for discharge results, SDD has the potential to impact the perception of quality care received by patients, promoting satisfaction while not compromising cardiovascular outcomes.
